From I-565:

Follow I-565 and exit at US 231 South, Memorial Parkway. Take the Airport Road exit off the Parkway and turn left at the stoplight. Go across the railroad tracks, then turn left at the 3rd traffic light onto Chateau Drive (Regions Bank is on the corner). The office is located within the Crestwood Professional Center building on the right, (250 Chateau Drive SW).

Co-pays are due at the time of service. There is a $10.00 "Billing Fee" that will be applied to your account if the co-pay is not paid at the time of service. This charge is not payable by your insurance and will be due from the patient.

​There is a $10.00 minimum fee for any paperwork/letters to be completed by our physician. This fee is not payable by your insurance company and will be due from the patient at the time of request.

Appointment Cancellation Policy

There is a $50.00 NO SHOW FEE for any missed appointments without a 24-hour notice. This fee is not payable by your insurance company and will be due from the patient.

New patients are requested to bring the following items to their scheduled appointment:

•       List of current medications
  
•       List of prior hospitalizations (if any), and approximate dates
  
•       Completed forms (Forms can be downloaded off our website, and are located under the Forms tab)

Driving Directions

There is a $30.00 fee for ALL returned checks.
